Group Information

If you are an internal staff member who will be submitting requests on your own behalf (i.e. not for a group, team, club, etc), use your first and last name as the Group Name.

Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

I acknowledge a Certificate of Insurance naming the Guilderland Central School District as an Additional Insured must be provided prior to approval.  The Guilderland Central School District must be named as an Additional Insured on a Primary and Non-Contributory basis.  Minimum acceptable limits for General Liability of $1,000,000 per Occurrence, $2,000,000 General Aggregate,  and $2,000,000 Products and Completed Aggregate must be provided.

 

The User of the Building agrees to defend and indemnify the Guilderland Central School District against any and all claims including those resulting in Bodily Injury, Personal Injury or Property Damage arising out of their use of the Building and grounds.

 

 

 

*Note: If the use of the premises involves any automobiles for demonstration,  exhibiting or deliveries of products or services, then Commercial Automobile insurance of at least $1,000,000 per Accident for all Owned, Hired or Non-Owned vehicles should be required with GCSD as an Additional Insured on a Primary and Non-Contributory basis as well.
